چکیده

Numerous iodide-containing compounds are used in radiographic contrast media. Severe, untoward reactions to these agents are of two types, cardiorespiratory collapse and, less commonly, anaphylactic reactions (Goodman and Gilman, 1965). This paper reports a case of severe laryngeal oedema which developed as an allergic reaction to sodium metrizoate 75 per cent (Triosil) administered during cardiac catheterization. Emergency laryngostomy was performed to prevent fatal asphyxia. Mullins et al. (1963) reported death due to asphyxia from laryngeal oedema after intravenous Hypaque (sodium diatrizoate), and Youngblood, Williams, and Tuggle (1956) a similar death after intravenous sodium acetrizoate, but survival of a patient under these circumstances has not been previously reported.

Advances in Anti-IgE Therapy

Omalizumab depletes free IgE in the blood and interstitial space and inhibits IgE binding to FcεRI on basophils, mast cells, and dendritic cells. We stopped omalizumab treatment after four years. Recurrences of urticaria symptoms were found to be higher in patients with chronic urticaria than recurrences of asthmatic symptoms in severe persistent asthma patients. For the very first time, we use...

Emergency use of the laryngeal mask airway in severe upper airway obstruction caused by supraglottic oedema.

We report two cases of severe upper airway obstruction caused by supraglottic oedema which developed rapidly at the time of anaesthesia. Conventional methods to relieve the obstruction failed and it was only overcome when a laryngeal mask airway (LMA) was inserted and positive pressure applied manually during inspiration. In one case a fibrescope was passed via the LMA and this revealed two cus...

Rapid Revival of a Patient after very Severe Metabolic Acidosis: A Case Report

Background: Metabolic acidosis is a fatal finding in trauma patients that complicates the process of resuscitation. Case: The case was a 37-year-old man with open fracture in both legs and fracture in second lumbar vertebral (L2). The serial arterial blood gas (ABG) test results showed a pH value of 6.7 indicating a very severe and special case of metabolic acidosis. The rate of mortality f...

Myxoedema presenting with severe laryngeal obstruction.

Introduction Thickening or oedema of the vocal cords and myxoedematous infiltration of the laryngeal submucosa may be responsible for the laryngeal symptoms of myxoedema in some patients, but respiratory difficulty due to laryngeal obstruction is extremely rare as a complication of myxoedema. If, however, upper respiratory tract infection is superimposed on a myxoedematous larynx an acute emerg...

Comparison of side effects during cerebral computed tomography with a nonionic (iohexol) and an ionic (metrizoate) contrast medium.

Cerebral computed tomography was performed in 60 patients referred for pre- and postcontrast scanning. The patients were randomly sampled with a nonionic contrast medium, iohexol, and an ionic contrast medium, sodium metrizoate. No complications were seen, and the adverse effects, usually a feeling of warmth, were very minor with both contrast media, but iohexol caused less discomfort than sodi...
